To leave you need to obtain a PIN number. You ask: How do I get it? Your agency will do for you that, but we need to bother ourselves and call the NMC and ask for this number personally.

Initially, they will give us number which is temporary. In the interview they ask us for the exact address to which you submit the form. Now, using our knowledge of English spells them all our address. Within 10 days of operational life we get a fairly thick  envelope with the form and accompanying instructions. - The Chamber of nurses and midwives need a certificate of professional nurses course,

- And a certificate of qualification to practice as a nurse in EU,

(For both of them you have to wait a long time about 2 weeks, as it was in  Warsaw)

To obtain documents from the Chamber of nursing need a certificate about the fact that over 3 years you have been paying contributions to the chamber of nurses, work certificates from establishments where you worked before, or a certificate if you still work in you last place.

All these documents must have your notarized copy, the cost per   1 page less than 8zł

In addition, you must perform all notarial translation documents with exception of the IC/ passport because it has its own translation and there is already an international recognized in other countries. The cost of such a translation is about 35 zł per page additional tax. Waiting for the document is about 4 days, unless we wish to express shipping service, if we assume an additional cost.

All those gathered documents we send to the agency, and they check them thoroughly with the annexed form. In the form, fill in lines on a history of education, all the qualifications, we write all of their employers. There are also heading there, that fills a physician, namely the signature and stamp and date, may even be a gynecologist, it's important to have a number assigned to phisician.

Funny questions are about your mental health, but I think that everyone part of themselves as healthy.

If someone is concerned   one column which must be submitted under oath, that you have a criminal record attached to the form completely replaces this subsection.

Even if you do not fill everything, your agency will do it for you properly, the only thing they can do is to sign up for you.

Now you can safely send it to the agent who will ask you to consent to any correspondence from the NMC is now coming into their address and they will continue to keep you informed about the case.

However, we decided to submit papers themselves,  if something was wrong, NMC does not refer back securities and does not inform about the failure.
